View our 65 Years of Stories campaign to learn more about the impact Benchmark employees have made across the country.Benchmark's South Bend office is seeking a part time Registered Nurse (RN) Healthcare Coordinator to provide healthcare coordination and direct intervention, as needed, for designated individuals in the Group Homes. The Nurse will also provide training for unlicensed staff to meet the healthcare needs of individuals served and contribute to the clinical oversight of program LPN, as applicable.Part-time position, averaging 20 hours per week, with a flexible schedule. Must be available for on-call support.Responsibilities:Provide healthcare and wellness coordination for individuals in accordance with all state and federal regulations.Function as liaison and advocate for individuals' healthcare with all members of individual medical and interdisciplinary teams.Advocate for client's wellbeing and inclusion.Train unlicensed staff on medication administration, delegated tasks, treatment protocols, and monitoringImplement and train employees on new and revised health related policies and proceduresParticipate in and attend assigned medical individuals' appointments (as directed), individual specific, nurse, departmental, and other meetings as required.Manage transitions of care to and from hospitals, nursing homes, rehab facilities or other agenciesOversee pharmacy and medication needsMaintain health-related records, complete documentation and assessmentsDevelop and update individual specific health risk plans to ensure appropriate medical care for assigned individualsDevelop long-term relationships with individuals servedProvide staff training to meet the healthcare needs of individuals serve.For a full and complete list, please contact HRQualifications:Current Indiana RN license.Valid Driver's License.Experience in Community/Long term careExperience working with individuals with intellectual developmental disabilities and/or mental illness.Requires being available for on-call support outside of regularly scheduled hours.Benchmark Human Services is an EOE/AAP Employer.
