Job Summary
We are seeking an experienced Case Manager Registered Nurse (RN) to join a growing acute care hospital team. This is a full-time permanent opportunity offering competitive pay, excellent benefits, and career growth.
Schedule
- Day Shift: 8:00 AM – 4:30 PM
- One weekend every 4 weeks
Compensation & Benefits
- Pay Range: $31.94 – $43.92/hr (based on experience)
- Sign-On Bonus: Up to $10,000
- Shift Differentials:
- Evenings: +$2.50/hr
- Weekends: +$2.00/hr
- Relocation Assistance Available

Responsibilities
- Perform patient assessments and develop individualized care plans
- Coordinate discharge planning and post-hospital services
- Review medical necessity, admission status, and resource utilization
- Collaborate with physicians, nurses, and interdisciplinary teams
- Conduct utilization reviews and communicate with insurance providers
- Identify barriers to care and facilitate quality patient outcomes
- Educate patients and families on available healthcare resources
- Promote efficient patient throughput and continuity of care
Requirements
- Active RN License (Compact License Accepted)
- ADN or Nursing Diploma Required
- BSN Preferred
- Minimum 3 years of recent Case Management experience in an acute care setting
