Role Overview: We are seeking licensed Physical Therapists to deliver personalized, one-on-one care to older adults in private homes and senior living communities, utilizing a flexible schedule and a signature Geriatric House Calls™ model. Key Responsibilities Provide evidence-based physical therapy to enhance mobility, independence, and overall well-being. Perform evaluations, develop individualized care plans, and deliver therapy sessions. Document patient progress accurately within the EMR system, adhering to regulatory standards. Collaborate with interdisciplinary teams including PTs, OTs, SLPs, and fitness specialists. Educate patients, caregivers, and staff on safety and aging-in-place strategies. Incorporate telehealth and virtual care to increase access and flexibility. Qualifications & Skills Valid Physical Therapy license and degree from an accredited program. Proficiency with computer systems and EMR documentation. Current CPR certification. Ability to meet physical demands such as lifting, repositioning patients, and driving.
