Responsibilities:

• Assist the PT in planning and implementing patient care and therapy services
• Assist patients with exercises, mobility, and activities of daily living (ADLs) as directed by the PT

• Educate patients and caregivers on proper techniques to improve mobility, reduce pain, and prevent injuries
• Maintain accurate and timely documentation of patient progress
• Collaborate with other healthcare professionals to ensure patients receive comprehensive care
• Provide home health and/or outpatient services as needed to ensure optimal outcomes

Qualifications:

• Must be a graduate of an accredited Physical Therapy Assistant program
• Must be licensed by the State as a Physical Therapy Assistant

• One year of home health experience required.
• Must have knowledge of Medicare and Medicaid guidelines
• Must have a valid driver’s license and reliable transportation.
• Must have a current CPR certification.
• Excellent verbal and written communication skills.
• Basic computer skill proficiency. Ability to use Microsoft Windows.

Background Screening Requirement:  Employment in Florida is contingent upon completion of a Level 2 background screening in accordance with Florida AHCA requirements. Applicants will be required to complete fingerprinting and background screening through the AHCA Clearinghouse.
