Personal Care Assistant

Employment Type: Full time Shift: Description: Position Purpose: The personal care assistant/worker is responsible for providing various care services necessary for attending to the personal needs, support, and health status of participants. Position Details: This position will primarily consist of home care assignments. However, occasional facility work will also be required. Position will require working one weekend a month and rotating holiday coverage. What you will do: * Engage in ongoing participant assessment and monitoring and report on services provided and the self-care abilities of participant. * Provide personal care assistance with activities of daily living, including toileting, showers, eating, and grooming. * Assist in the maintenance of physical activity, exercise, and walking. * Identification of safety hazards and emergency situations and initiating corrective action as able. * Assist with general activities such as conversing with participants, recreation activities, meal and snack serving, and housekeeping. * Assist in identifying signs and symptoms indicating physical, emotional, or behavioral change in participants and reporting these signs to the center staff and/or supervisor * Escort participants to appointments as necessary. * Assist transportation staff with loading and unloading of participants from vehicles. Assist participants being transported in PO vehicles as needed. Minimum Qualifications: * High School Diploma or equivalent required. * Minimum of one (1) year of documented experience working with a frail or elderly population or if less than one (1) year of experience, must receive appropriate training from the PACE organization on working with a frail or elderly population upon hiring. * Experience in a long-term care facility or with a community-based geriatric program preferred. * Must be action-oriented, manage conflict well, be customer focused, have high decision quality and flexibility to adapt to ongoing change. * Ability to work with minimal supervision and exercise independent judgment. * Superior written and verbal communication and presentation skills appropriate for audience comprehension. * Valid drivers license and use of personal vehicle for home care visits * Excellent organizational skills and the ability to perform multiple duties and functions related to daily operations while maintaining excellent customer service. * Position may require 10% travel within the Centers geographic region and use of personal vehicle.
