This role offers the opportunity to manage a full caseload while working closely with an interdisciplinary team dedicated to high-quality hospice care.OverviewThe Hospice RN Case Manager provides skilled nursing assessment, planning, and care to hospice patients and their families. The RNCM manages an assigned caseload and collaborates with interdisciplinary team members to deliver compassionate, effective care aligned with patient goals and hospice standards.Key ResponsibilitiesComplete initial and ongoing assessments of patient physical, emotional, psychosocial, and environmental needsCoordinate with physicians for certification and recertification of terminal illness in compliance with Medicare regulationsServe as the primary clinical contact for assigned patients, including care planning, symptom escalation, and advocacyCoordinate care with all providers and support systems to ensure seamless hospice servicesDevelop and update individualized plans of care in collaboration with patients, families, and the interdisciplinary groupProvide skilled nursing care in accordance with the plan of care and document interventions and outcomes promptlyEnsure documentation supports regulatory, quality, and billing compliance requirementsEducate patients and families on disease progression, symptom management, medications, and hospice philosophySupervise hospice aides and support the development and revision of aide care plansParticipate in interdisciplinary group meetings and coordinate with physicians, social workers, chaplains, and bereavement staffCommunicate effectively with interdisciplinary team members and clinical leadershipParticipate in the on-call rotation and respond to urgent patient needs as requiredEscalate care needs to the medical director or covering provider as clinically indicatedPerform other duties as assignedQualificationsGraduate of an accredited nursing program; Associate's degree or higher preferredCurrent RN license in the state of practice or valid multistate compact licenseMinimum of one (1) year of hospice, palliative care, or home health nursing experience preferredCPR certification and valid driver's license requiredStrong clinical judgment, documentation accuracy, and communication skillsKnowledge of hospice regulations and Medicare Conditions of Participation preferredAbility to work independently while collaborating across interdisciplinary teamsCommitment to Arc Hospice & Palliative Care's mission and valuesThe starting compensation for this position is shown below. The rate displayed reflects the starting hourly or salaried pay for the role and does not represent a range.Final pay is determined based on factors such as experience, qualifications, location, and business needs at the time of hire.
