Social Worker Job Summary:
We are seeking a compassionate and skilled Social Worker to join our dedicated team of healthcare professionals. As a Social Worker at our facility, you will play a vital role in providing emotional support and connecting our residents with the resources they need to thrive. Social Worker Responsibilities:
Assess the social, emotional, and psychological needs of our residents, providing personalized support and interventions as needed
Develop and implement comprehensive care plans in collaboration with the interdisciplinary team
Facilitate connections with community resources, services, and organizations to support residents' well-being and independence
Provide emotional support and counseling to residents, families, and staff members
Document resident interactions, assessments, and treatment plans in accordance with established policies and procedures
Participate in quality improvement initiatives and contribute to the development of best practices in social work services
Social Worker Requirements:
MSW preferred, Bachelor's degree in Social Work (BSW) also accepted
Licensure as a Social Worker in the state of New York (LSW or LCSW)
