Key Highlights Weekly pay: $2,513 to $2,638 per week Guaranteed hours: 40 hours per week Contract duration: 14 weeks Shift: 8-hour days Start date: 09/21/2026 Work setting: home health / hospice care in patient residences Fast credentialing and dedicated support throughout the assignment Housing assistance and 24/7 recruiter/travel support Extension opportunities Responsibilities Assess home health and hospice patients in stable and emergent situations to identify physical, psychosocial, and environmental needs; document findings for care planning. Develop, implement, document, and evaluate the Plan of Care to ensure quality and continuity of care. Obtain data on physical, psychological, social, spiritual, and cultural factors influencing patient/family health status and incorporate this data into the Plan of Care. Maintain up-to-date patient records with visits, contacts, problems, plans, actions, and goals; reflect changes as they occur. Apply infection control practices to protect staff and patients; educate patients/families on care, medications, home safety, and related topics with culturally appropriate teaching methods. Supervise and provide clinical directions to HHA/STNA/LVN staff to ensure quality and continuity of service. Assess and reassess pain; utilize appropriate pain management techniques and educate patient and family on pain control strategies. Arrange referrals to appropriate agencies when needed; monitor compliance with third-party payer requirements. Perform on-call responsibilities and provide on-call services as assigned; maintain effective communication with patients, families, team members, and other health professionals. Attend interdisciplinary team meetings and other required meetings; support coordination of care across care settings.
