Clinical Nurse

The Clinical Nurse is a Registered Professional Nurse who provides safe, competent quality care based on nursing theory and research to a designated group of patients and significant others. Delivers patient care by incorporating the principles of Relationship Centered Care and applying the nursing process based upon theory and evidence-based practice. Assesses and evaluates patient care needs while applying critical thinking skills in patient management. Integrates relevant assessment and intervention skills in the delivery of nursing care. Collaborates with the interdisciplinary healthcare team to develop, implement, and evaluate the plan of care. Prioritizes all aspects of patient care including teaching, rounding, coaching, and planning, while delegating appropriately. Educates patients and caregivers proactively about their plan of care, transition of care, health promotion, and disease prevention. Communicates effectively and professionally with patients, families, and healthcare team members. Manages assignments within the model of Modified Primary Nursing and demonstrates effective allocation of resources. Role models the tenets of Relationship Centered Care through various activities focused on patient experience. Considers patients' values, preferences, cultural diversity, expressed needs, and knowledge in all aspects of care. Establishes and maintains therapeutic relationships with patients and families. Utilizes evidence-based practices to enhance understanding of patients' perceptions of care. Initiates service recovery for patients and families who have concerns, escalating as needed. Demonstrates caring, respect, compassion, empathy, and active listening through dialogue and actions. Conducts purposeful rounding on assigned patients to address basic needs. Practices safe and efficient patient-centered care while identifying opportunities for improvement. Demonstrates critical clinical skills and knowledge essential for quality and safety in the patient population served. Implements improvement strategies based on institutional and evidence-based practices. Engages in peer and institutional review processes. Collaborates with interdisciplinary teams to create and implement quality improvement projects and nursing research studies. Contributes to quality and safety practices and compliance measures. Implements principles of high reliability to avoid high-risk behaviors and ensure a safe environment. Escalates potential safety hazards or gaps in practice following established protocols. Utilizes institutional information technology by documenting nursing practice to support quality initiatives. Performs charge nurse duties as assigned. Coordinates with clinical leadership to ensure appropriate utilization of supplies and inventory for patient care. Escalates resource deficits to appropriate personnel. Demonstrates patient-focused and cost-effective approaches to patient care. Documents accurately and thoroughly in compliance with standards of care. Facilitates patient throughput on a shift basis. Contributes to an environment that supports clinical colleagues, patients, and families. Advances clinical competence in nursing practice to progress from novice to expert. Projects a professional image to colleagues and communicates with compassion and empathy. Incorporates ethical principles into decision-making for patients and families. Encourages and demonstrates a commitment to scholarship, inquiry, and lifelong learning. Acquires knowledge and skills relevant to the role, patient population, and clinical specialty. Participates in shared decision-making through practice councils, committees, and unit initiatives.
