We are seeking an experienced Physical Therapist (PT) for a 12-week contract position in Carlinville, Illinois. This outpatient rehabilitation role focuses on evaluating patients, developing individualized treatment plans, and providing evidence-based therapy to improve mobility, function, strength, and independence.

The Physical Therapist will work collaboratively with physicians and rehabilitation professionals while providing patient-centered care throughout the rehabilitation process.

• Perform comprehensive physical therapy evaluations and develop individualized treatment plans based on patient needs and goals.

• Provide evidence-based outpatient rehabilitation interventions to improve mobility, strength, balance, flexibility, and functional independence.

• Monitor patient progress, reassess functional outcomes, and modify treatment plans as clinically appropriate.

• Maintain accurate clinical documentation and complete required records according to facility and billing guidelines.

• Collaborate with physicians and interdisciplinary rehabilitation professionals to coordinate patient-centered care.

• Active Illinois Physical Therapist License Required.

• Minimum 1 Year of Physical Therapist Experience Required.

• Outpatient Rehabilitation Experience Preferred.

• CPSI/Evident Experience Preferred.

• Strong Clinical Assessment, Communication, Documentation, and Patient Education Skills Required.

The Physical Therapist will provide individualized outpatient rehabilitation services focused on improving patient mobility, function, and independence.
