Your Primary Responsibilities
- Carrying out thorough patient evaluations, requesting necessary diagnostic tests, and developing personalized care strategies
- Diagnosing and managing acute, episodic, or chronic conditions in collaboration with medical staff, or autonomously (if credentialed)
- Guiding patients and their families through preventive care, wellness education, and chronic disease management
- Prescribing necessary medications and treatment plans within your practice scope
- Engaging in follow-up care and managing chronic health concerns
- Working collaboratively with interdisciplinary teams to ensure comprehensive, cohesive patient care
- Accurately documenting all medical interactions using electronic medical records (EMR)
- Master of Science in Nursing (MSN) or Doctor of Nursing Practice (DNP)
- Completion of an accredited Nurse Practitioner training program
- Active Nurse Practitioner license in the state of Wisconsin
- National certification as a Family Nurse Practitioner (FNP-C, AANP, or ANCC) or similar qualification
- Basic Life Support (BLS) and Advanced Cardiovascular Life Support (ACLS) certifications (or ability to obtain within 90 days)
- Solid clinical judgment paired with a warm, empathetic approach
- Exceptional communication abilities with patients, family members, and fellow team members

- Proficiency in utilizing EMR systems and providing care via telehealth platforms
