Description Job Title: Physical Therapist Location: Winton Road Site, Rochester, NY 14623 Signing Bonus: Eligible for a $6,000 signing bonus earned in four installments: 25% ($1,500) in the pay period following the 6‑month anniversary of employment; 25% ($1,500) in the pay period following the one‑year anniversary; and 50% ($3,000) in the pay period following the two‑year anniversary. Annual Salary: $66,000—$80,000 commensurate with experience Hours of Employment: Full‑time, 40.0 hours per week. Days and hours of work are Monday through Friday, variable each day between the hours of 8:00 am–5:30 pm. Evening work may be required as job duties demand. Requirements Bachelor’s degree in Applied Science. Completion of an accredited Physical Therapy program. License or eligible for license as a Physical Therapist in New York State. Current New York State registration as a Physical Therapist. Valid New York State driver’s license. Experience working with individuals with developmental disabilities preferred. Proficient computer skills with knowledge of Microsoft Office programs. Demonstrated commitment to performing job duties and tasks at a high level of performance. Summary / Objective CP Rochester/Happiness House exists to support people with physical and developmental disabilities in choosing and accomplishing progressive, individualized life goals. We provide a wide range of services to people of all ages in the Finger Lakes and Greater Rochester area. Our purpose is to ensure that those we serve safely determine their own pathways in life and live as full members of their communities. Essential Functions Imparts agency philosophy of “Equal Opportunity, Independence and Realization of Individual Potential” to children, families, program staff and community resources. Maintains confidentiality of information relating to individuals and their families in accordance with agency policy. Upholds NYS Education Law/Practice Act as it relates to the practice of physical therapy. Performs appropriate evaluations to determine the need for physical therapy services. Ensures valid prescriptions are written for all individuals on caseload. Delivers treatment plans developed after evaluation, in clinic setting for the Article 16 and Article 28 outpatient clinics as well as the Preschool or School Programs when necessary. Consults with other professionals such as teachers, habilitation staff, and other interdisciplinary team members to facilitate coordinated quality services. Works with legal guardians, house staff or parents as integral members of the interdisciplinary team in clinic, day‑hab or school setting when needed. Communicates with physicians, orthopedists and other medical personnel to facilitate evaluation and treatment. Completes all paperwork in accordance with agency, program and department policies and guidelines. Ensures knowledge of paperwork, Electronic Health Record and applicable Medicare and Medicaid regulations for appropriate documentation. Attends all team, discipline and staff meetings as designated. Maintains treatment area in an orderly manner to ensure safety. Maintains all department equipment appropriately. Advise Supervisor of Clinical staff of need for replacement or ordering of new equipment. Meets with interdisciplinary team and Program Supervisor to review status and need of program modifications for individuals on caseload. Participates in conferences, workshops and in‑service training as needed or recommended to enhance job skills and personal growth and development. Adheres to agency’s policies and procedures as well as department guidelines. Competency Requirements Client focus. Strong collaboration skills. Communication proficiency. Ethical conduct. Remaining in compliance with mandatory trainings per agency policy and applicable government regulations. Maintains CPR and First Aid Certification. Supervisory Responsibility None. Potential to supervise Physical Therapy Students from local colleges and universities. Physical Demands Regularly required to talk or hear; sit, stand, walk; use hand or arm to finger, handle or feel; reach with hands and arms. Must frequently lift and/or move objects up to 10 lbs and occasionally up to 50 lbs. Ability to pull and push wheelchairs up to 250–300 lbs (maximum). Travel Primarily local during the business day but may be required to work at partner agencies. Some out‑of‑area and overnight travel may be expected.
