Responsibilities
- Provide direct nursing care to residents in a long-term care setting, including assessment, planning, and implementation of individualized care plans.
- Administer medications and treatments as ordered by physicians and in accordance with facility policies.
- Monitor residents’ conditions, document vital signs and changes, and communicate findings to the interdisciplinary team.
- Assist residents with activities of daily living and mobility, supporting safety and dignity.
- Respond to call bells and emergencies promptly, following established emergency procedures and infection control practices.
- Collaborate with charge nurses, coordinators, and other care team members to ensure high-quality, resident-centered care.

Requirements
- Active Licensed Practical Nurse license in Vermont (in good standing)
- Ability to work across all shifts in a long-term care setting
- Strong communication and documentation skills; reliable team collaboration
- Willingness to meet travel assignment requirements and comply with relevant regulations and facility policies
