Field-Based ECM Care Lead & Community Advocate
Pacific Health Group
Pacific Health Group is seeking a Lead Care Coordinator to support members in the community through field-based visits within the hiring county. You will develop care plans, coordinate services, and advocate for timely access to care while navigating housing, food access, transportation, and behavioral health needs.
