Seven Healthcare are seeking a dedicated Physical Therapist for a contract assignment in Florence, SC. This role will primarily involve providing physical therapy evaluations and treatments in a home health setting. Join a supportive work environment that values your expertise and dedication.

Key Responsibilities of the Physical Therapist:
- Perform physical therapy evaluation and treatment of patients based upon physician orders/referrals
- Complete comprehensive evaluations using appropriate tools and procedures
- Review patient records for current diagnosis, past medical history, precautions, and contraindications
- Establish measurable goals and develop treatment plans
- Regularly reassess treatment plan effectiveness and patient progress
- Communicate patient needs and progress to the treatment team and relevant parties
- Consult with medical providers regarding patient evaluation, treatment, and discharge
- Recommend and assist in securing special adaptive equipment
- Maintain and submit all required documentation timely and accurately
- Adhere to all applicable standards and regulations
- Supervise Physical Therapist Assistants and Aides
- Participate in staff development programs and maintain clinical practice skills
Requirements of the Physical Therapist:
- Must have a valid Physical Therapy license
- Proficiency in Home Health
- Experience with Home Care Home Base documentation
