Responsibilities
- Deliver direct patient care to residents per individualized care plans and facility policies
- Administer prescribed medications and treatments; ensure accurate documentation
- Monitor vital signs and general health status; recognize and report changes promptly
- Respond to call lights; assist with activities of daily living and mobility as needed
- Collaborate with the interdisciplinary team to update care plans and ensure safety
- Maintain accurate records in the electronic health record and uphold infection control standards

Requirements
- Active Licensed Practical Nurse (LPN) license
- Local candidates only; ability to work in Albuquerque, NM
- Availability for night shift (12-hour shifts) and 3x weekly schedule
- Experience in long-term care or skilled nursing settings preferred
- Strong communication, documentation, and teamwork skills
- Basic life support (BLS) certification preferred
