Registered Nurse (RN) Case Manager

Job Summary

The RN Case Manager is responsible for providing skilled nursing care and comprehensive case management for assigned patients in their homes. This role involves admitting patients to services, coordinating care, and ensuring seamless communication across healthcare providers to facilitate high-quality clinical and cost outcomes. The RN Case Manager plays a crucial role in evaluating patient needs, developing care plans, and providing education and support to patients and their families.

Essential Duties and Responsibilities

• Deliver skilled nursing and coordinate care for patients and their families in a residential environment using a team-oriented approach.
• Conduct thorough assessments of patients' physical and psychosocial conditions, planning and implementing appropriate interventions as authorized by a physician.
• Oversee care provided by Home Health Aides and CNAs, ensuring compliance with established care plans.
• Facilitate continuity of care through effective communication, information sharing, and participation in case conferences with interdisciplinary teams and community health agencies.
• Provide quality care and manage symptoms effectively to promote positive clinical outcomes and enhance patients' quality of life through education and appropriate referrals.
• Maintain accurate and up-to-date patient records in accordance with agency standards and regulatory requirements.
• Participate in team scheduling and on-call rotations as necessary to meet patient care demands, assessing patient needs via telephone and making home visits when required.
• Engage in quality improvement initiatives and participate in in-service training sessions as assigned.
• Perform additional duties as directed by supervisory personnel.

Minimum Qualifications

• Current RN license in the State of Montana is required (BSN preferred).
• Valid driver's license, appropriate auto insurance, and reliable transportation are required.
• Two years of recent acute care, home care, or hospice experience, including clinical skills such as venipuncture, catheterization, wound care, and parenteral therapy, is preferred.
• Experience with HCHB is strongly preferred.

Schedule

This position typically follows a Monday to Friday schedule from 8:00 AM to 5:00 PM, with required on-call and weekend rotation to meet patient care needs.
