Geriatric Occupational Therapist - Rehab & Care Team Lead

The Chatham Hills Subacute Care Center is seeking a dedicated Occupational Therapist to join our rehab team in New Jersey. You will assess referred patients, develop and administer individualized treatment plans, and set measurable goals in collaboration with physicians.
You will document progress, educate patients and families, and supervise OT Assistants while coordinating with other disciplines to ensure excellent geriatric care. Weekdays and weekends shift needs are noted.
