Hospice Registered Nurse Case Manager

Community Healthcare of Texas is recruiting an RN Case Manager for a full-time role in Dallas, TX. The Case Manager meets with patients and families to explain hospice services, coordinates admission, and develops the ongoing plan of care. Essential duties include providing nursing care, assessing and revising care plans, administering medications, coordinating with physicians and the hospice interdisciplinary team, documenting notes, and supervising LVN/CNA care plans. #J-18808-Ljbffr
