Skilled Nursing Facility Physical Therapist | Minden, Louisiana

Step into a rewarding opportunity as a Physical Therapist serving students across multiple campuses in the Homer, LA area for the 2026-2027 academic year. This contract role offers a unique part-time schedule-just two days per month-allowing you to make a valuable impact while maintaining excellent work-life balance. With campuses spanning three towns no more than 15-30 minutes apart, your skills will benefit a diverse student population from PreK to 12th grade.

Qualifications/Desired Experience:

Valid Physical Therapist license in Louisiana
Experience working in pediatric, educational, or school settings preferred
Knowledge and understanding of Louisiana Bulletin 1508 PT evaluation standards
Strong assessment, documentation, and organizational skills
Comfort with independent travel between school sites
Ability to collaborate with multidisciplinary teams

Key Responsibilities:

Conduct comprehensive initial and re-evaluations in accordance with Louisiana Bulletin 1508
Assess students' physical therapy needs from PreK through high school
Complete thorough documentation and provide professional recommendations as required for Individualized Education Programs (IEPs)
Collaborate with educational staff and families to share findings and support student needs
Maintain compliance with district policies and state guidelines
Travel efficiently between 6 school campuses in three nearby towns
