Job Summary and Responsibilities Dignity Health Medical Group - Arizona is seeking a full-time Geriatric Nurse Practitioner to join our busy department in Prescott, Arizona EMR: Cerner This position will be Alzheimers and Dementia Care focus. Project volume: 250 patients Office Patients: Patients are seen in the office with some off-site home visits in the surrounding area Typical work week (hours worked; hospital vs. clinic) - option to work 4 or 5 days per week. Working in clinic setting, as well as, off site visits throughout the community to accommodate the patients needs. Typical patient visit load of 2 new consults a day and f/u visit Job Requirements Current Board Certification Valid medical license Valid DEA certificate 2-5 years experience in geriatrics with focus on cognitive diseases preferred Where You'll Work Yavapai Regional Medical Center was incorporated in 1942 and affiliated with Dignity Health in Arizona in 2020.
