Licensed Practical Nurse (LPN) - LPN

LPN/LVN Home Health

Schedule: 8 hours

Assignment Details:

• Contract Length: 13 weeks
• Guaranteed Hours: 40

Requirements:

• Current Nursing License in the Practicing State
• Valid Driver's License
• Current CPR Certification
• Two years of experience as an LPN/LVN in a clinical setting, preferably in a home health or hospice setting
• Proficiency in using electronic medical charting software

Roles & Responsibilities:

• Administer skilled nursing care to patients in their residence as prescribed by the physician.
• Assist in identifying and assessing patients' physical, psycho-social, and environmental needs under the direction of the RN.
• Cooperate with the RN in planning, coordinating, and implementing patient care to ensure quality and effective outcomes.
• Communicate effectively with patients, families, and the healthcare team regarding care and outcomes.
• Educate patients and families on self-care techniques and assess their responses to the care provided.
• Adhere to infection control protocols to ensure the safety of both patients and staff.
• Support end-of-life care for Hospice patients when required.

Compliance / Notes:

• Must participate in annual in-service and performance improvement programs.
• Proof of current Hepatitis consent/declination and TB vaccination record required.
• Reliable transportation and current auto liability insurance coverage necessary.
• Ability to maintain professional boundaries in relationships with co-workers, patients, and caregivers.
