Registered Nurse at Sunrise Senior Living Edison, NJ

Overview When you join Sunrise Senior Living, you will be able to use your unique skills to empower residents to live longer, healthier, and happier lives. This role involves building meaningful relationships with residents, their families, and team members, and contributing to a culture of service and quality care. Responsibilities Clinical Care: Assist with the development and leadership of the Designated Nursing Assistants (DNA)/Certified Nursing Assistants (C.N.A); follow the Resident Centered Care model. Assist with case management of both Medicare and non-Medicare guests/residents as needed. Participate in Care Planning meetings. Provide clinical care including urinary cauterization, tube feedings, applying/changing dressings, Accu-checks, medication management/administration, admission, transfer and discharge of guests/residents. Ensure interdisciplinary notes reflect the care provided through daily audits. Complete timely and accurate documentation of guests/residents charts. Complete Incident Reports and report to the Director of Nursing Services (DNS). Review, update and document communication regarding residents to learn and share pertinent resident and community information. Conduct effective visits/rounds at a minimum twice a day; conduct visits/rounds with physicians as necessary as assigned by DNS. Research and document incidents of abuse, neglect and misappropriation of funds as assigned by DNS or Executive Director, Skilled Nursing Administrator. Actively participate in Clinical IDT Meetings and Daily PPS meetings to obtain information on admissions and discharges. Assist in monitoring medication records and administration. Assist in receiving lab, x-ray and other test results and communicate appropriately. Quality Assurance & Regulatory Compliance: Strive for excellent quality care and service delivery as measured by the 5 Star quality measures; demonstrate knowledge of standard practices and resident care and services. Demonstrate knowledge in: skilled nursing transformation and Resident Centered Care Model, Sunrise and Federal Quality Indicator; Federal, State/Provincial and Local resident care nursing regulations; Resident Rights; Resident Assessment Instrument process; participate in Federal, State/Province, Local surveys. Restorative: Maintain knowledge of Restorative Practices of the RAI process; lead and follow documentation guidelines for the Restorative process; monitor documentation for the Restorative program; monitor resident decline/improvement; refer residents to restorative nursing as indicated. Clinical Systems: Demonstrate Sunrise practices for care and services relating to skin, nutrition, behavior, falls, bowel/bladder; assist in implementation, training and monitoring of documentation related to clinical systems weekly; consult with physicians as appropriate; ensure guest/resident changes, concerns and/or solutions are communicated to the DNS as appropriate. Infection Control: Understand and follow infection control practices; provide training as needed; follow proper methods for exposure to bloodborne pathogens and hazardous materials in compliance with universal precautions; report occupational exposures immediately; ensure oxygen tanks are stored safely and maintained at prescribed levels. Medication Management: Transcribe physician orders accurately to eMAR; demonstrate knowledge of the Six Rights of Medication Pass; maintain knowledge of medications for the senior population; provide leadership and follow Sunrise/nursing standards for taking orders and administering medications; ensure current resident photos and dates on eMAR; complete eMAR audits monthly; monitor medication rooms, charts, and treatment carts for organization and medication freshness. Guest/Resident, Family & Visitor Relations: Respond to resident, family and visitor requests in a timely and professional manner; communicate with families regarding medical care; coordinate resources; maintain confidentiality; review 24-hour SCC dashboard and high-priority progress notes. Risk Management & Safety: Ensure compliance with occupational health and safety regulations; promote risk management programs; adhere to safety rules; participate in safety training (PPE, SDS, Lockout/Tagout); drivers must follow the Driver job description where applicable. Training & Team Contribution: Actively participate as a team member; demonstrate the Team Member Credo and Principles of Service; contribute to engagement activities; attend regular meetings; assist with supervising team members (hiring, orientation, training, performance evaluations); maintain required trainings to ensure Sunrise standards; perform other duties as assigned. Experience & Qualifications Graduate of an approved college/school of nursing Maintains a current state/provincial RN license Minimum of one (1) year supervisory/management experience in a skilled nursing, long-term care, hospital or home health environment (including hiring, coaching, discipline, performance management, and daily operations supervision) Demonstrated knowledge of nursing practices and methods in skilled nursing; knowledge of federal/state/provincial long-term care regulations Understanding of the Resident Assessment Instrument (RAI) and Care Plan Process Ability to handle multiple priorities and delegate effectively based on skills, roles, and interests Strong written and verbal communication skills; ability to educate and guide clinical team members Strong organizational and time-management skills; good judgment, problem solving and decision-making abilities Proficiency in computer skills, Microsoft Office (Windows, Outlook, Excel) and Sunrise applications; ability to learn new applications Ability to work weekends, evenings and flexible hours according to service demands #J-18808-Ljbffr Neier Inc.
