Role: Physical Therapist (PT) Schedule: Full-time Monday-Friday + occasional weekends (as needed) Location: Field Based Compensation (Productivity Points): $75.00 / point | $100,000-$130,000/year We're fixing home healthcare, and we're building our presence in Detroit Adaptive is building an AI-native home healthcare platform that enables clinicians to focus on delivering best-in-class care for their patients by leveraging software and innovative support solutions. We’re building our team in Detroit with plans to quickly develop an exceptional reputation of delivering excellent patient care. The mission of this role Deliver excellent home health care to our patients in Detroit, and set the clinical bar, the documentation standard, and the referral-source trust that our market will be built on. Objectives Carry a full, high-quality caseload: ramp to ~28 productivity points/week at target visit quality. Own your patients' outcomes: drive low avoidable-hospitalization rates and strong patient-experience scores; participate in case conferences/team huddles to improve our quality. Be a referral-source ambassador: every discharge planner, ALF, and physician you touch should want to send the next patient to us because you were responsive and excellent. Set the standard others copy: your charting, cadence, and judgement become the template we onboard the next cohort against as the pod grows. What you'll own Own the caseload as a case manager: you’re accountable for the patient’s therapy plan, progression, and outcomes - not just individual visits. Complete comprehensive in-home assessments and deliver skilled PT visits. Build and execute a clear plan of care: gait/balance, transfers, strengthening, functional mobility, home safety, fall prevention. Set measurable goals, track objective progress, and adjust treatment quickly when patients stall. Drive multidisciplinary coordination: communicate changes in condition, align on orders, and keep the care team moving. Manage treatment and outcomes directly, including PTA supervision where appropriate. Coach patients and caregivers with practical home programs that actually get done. Document promptly and thoroughly in Homecare Homebase (HCHB) - defensible, efficient, and consistent. Must-haves Active Michigan PT license (in good standing) Strong clinical judgment and comfort working independently in the field Comfortable escalating changes in condition to RN/MD and documenting rationale clearly Strong communication and documentation Reliable follow-through on orders, frequencies, and patient goals Current driver’s license and comfort with daily travel across assigned territory Able to carry/transport basic therapy equipment as needed Competency in core home health PT scope Safety-first mobility work: transfers, gait training, balance/fall prevention, assistive device recommendations Strengthening, functional mobility, and home safety assessment/modification education Patient/caregiver coaching with practical home programs Nice-to-haves Prior home health experience Homecare Homebase (HCHB) experience (or similar home health EMR) Deeper clinical experience in high-signal PT populations Experience supervising/mentoring PTAs and collaborating on care plans Strong organization and time management Our guarantee Full-time target: ~28 productivity points/week ; clinicians earn more for exceeding it. Guaranteed floor: ~24 points/week for the first 90 days - if your caseload hasn’t filled, you’re still paid the floor. The rest of the package
