Moms In Motion - A Virginia Service Facilitation Provider
Moms In Motion is seeking a Service Facilitator (SF) Case Manager to support Virginia families enrolled in Medicaid Waiver programs. You will visit clients in their homes, coordinate plans of care, and help families navigate CCC+, CL & FIS, EPSDT requirements. No degree is required; at least 2 years of experience with disabilities or elderly care is preferred. You’ll travel locally and work from a laptop to coordinate services and communicate clearly with families. #J-18808-Ljbffr
