Social Worker needed for full-time, temp to hire position in Camden County. The Social Worker will provide services to support residents' social and emotional well-being, ensuring they achieve their highest level of function. Responsibilities include developing and implementing social service policies in compliance with regulations, conducting care conferences, discharge planning, and facilitating resident and family activities. The role involves counseling residents and families on emotional and end-of-life concerns, completing MDS assessments, and maintaining accurate documentation. The social worker will coordinate with community agencies, assist with financial and emotional security resources, and support resident rights. Additional duties include organizing in-service programs, infection control, quality assurance initiatives, and collaborative care planning with facility staff.

NJ CSW or LSW with long-term care experience.
