Responsibilities
- Perform comprehensive physical therapy evaluations and assessments
- Develop and implement individualized treatment plans based on patient goals
- Deliver hands-on therapy using evidence-based techniques
- Educate patients and caregivers on exercises, safety, and recovery strategies
- Collaborate with interdisciplinary healthcare professionals to ensure coordinated care
- Maintain accurate and timely documentation using EMR systems
- Participate in discharge planning and continuity of care
- Adults and Pediatrics
- Geriatrics
- Orthopedics
- Home and community-based care
- Licensed Physical Therapist or eligible for licensure
- Strong knowledge of anatomy, physiology, and rehabilitation principles
- Experience in outpatient, inpatient, or home health settings preferred
- Ability to work independently in a home health environment
- Excellent communication and patient education skills
- Caseload of 3-5 patients per day
- Flexible scheduling including day, night, and on-call shifts
- Weekend availability optional and preferred
- In-person, home-based care setting
