Job Description - Hospice Registered Nurse (RN) Case Manager (2606342) Registered Nurse Case Manager responsible for evaluating, assessing, and treating patients in accordance with policy and individualized care plans; ensuring accurate documentation; following policies and procedures; maintaining continuity of patient care in a hospice setting.
