Humana's Primary Care Organization is a provider of senior-focused, value-based care, with more than 340 centers across 15 states under the CenterWell and Conviva brands. This is a full-time, on-site, patient-facing role. You report to the Associate Medical Director and are responsible for both in-visit care and ongoing patient management. You will provide comprehensive primary care to senior patients within a value-based care model that emphasizes quality outcomes, preventive medicine, and long-term health management.

What You'll Do

• You will see 12-15 patients per day, allowing for longer, more in-depth visits
• You will manage patients with complex and chronic conditions
• You will complete accurate, same-day documentation to support patient care and outcomes
• You will manage a daily clinical inbox (labs, messages, follow-ups)
• You will coordinate care with specialists and care team members
• You will apply Medicare risk adjustment principles through accurate condition documentation

Use Your Skills to Make an Impact

Required Qualifications

• MD or DO from an accredited program
• Board Certified or Board Eligible in Family Medicine, Internal Medicine, or Geriatrics
• Active, unrestricted medical license (or ability to obtain) in South Carolina

Additional Experience We Value

• 2+ years of clinical experience with senior populations
• Experience with Medicare Advantage or risk adjustment
• Experience supervising Advanced Practice Providers (NPs/PAs)
• Proficiency with electronic health records (EHR) and digital documentation.
