Occupational Therapist – Travel assignment in Puyallup, Washington. This 13-week contract begins 06/01/2026 with guaranteed 35 hours per week and a 5x8 schedule. You’ll earn a weekly pay range of $2,004 to $2,072. The role is in a long-term care nursing home setting and involves a shared, multi-site rotation within the region (three days at one facility, two days at another each week). Responsibilities
- Assess residents’ functional abilities and develop individualized OT treatment plans to improve activities of daily living and overall independence.
- Implement evidence-based therapies to address physical, cognitive, and psychosocial goals; monitor progress and adjust plans as needed.
- Perform routine documentation, progress notes, and outcome measures in accordance with facility policies and regulatory requirements.
- Collaborate with an interdisciplinary team to coordinate care, communicate changes in plan of care, and support discharge planning when appropriate.
- Provide patient and family education on home exercise programs, safe activity performance, and fall prevention.
- Manage a shared schedule across two facilities, with a weekly rotation that supports coverage and continuity of care.

Requirements
- Active Occupational Therapist license in Washington state.
- Minimum 2 years of OT experience; long-term care experience strongly preferred.
- Ability to work in a multi-site, shared assignment with a split weekly schedule (3 days at one facility, 2 days at another).
- Strong communication, documentation, and collaboration skills; proficient with standard electronic medical records.
- Ability to meet guaranteed hours (35 per week) and adapt to a 5x8 day shift.
