OT - Occupational Therapist

Job Summary An Occupational Therapist (OT) administers occupational therapy to patients on an intermittent basis in their place of residence. This is performed in accordance with physician orders and plan of care under the direction and supervision of the Director of Clinical Services/Clinical Manager. Qualifications Graduate of an Occupational Therapy curriculum accredited by the Committee on Allied Health Education and Accreditation of the American Medical Association and the American Occupational Therapy Association (AOTA) and/or certified by successfully completing the National Certification examination, or is eligible for the National Registration Examination of the American Occupational Therapy Association. Currently licensed in Florida with one year’s experience preferred. Must have current CPR certification, TB test, Health Statement/physically clear to work, and eligible Level 2 background. Responsibilities Improve or minimize residual physical disabilities of the patient. Return the individual to optimum and productive living within their capabilities. Periodically participate with all other home care personnel in patient care planning. Provide prescribed occupational therapy. Direct and supervise personnel as required. Conduct initial assessment, plan of care, maintenance program development, modifications, and reassessments. Perform all skilled procedures. Consult physicians regarding changes in treatment. Instruct patients/family members in in‑home programs and fine motor movement exercises. Complete initial evaluation, including a plan of care and goals, and submit to the physician for approval. Recertify by a physician every 60 days if treatment continues. Write a progress note each visit, a reevaluation when expected duration is reached, and a summary upon discharge. Assist the physician in evaluating the level of function. Help develop the plan of care and revise as necessary. Prepare clinical and progress notes. Advise and consult with family and other agency personnel. Provide one‑on‑one patient care.
