Call Schedule: 1:4 - predictable and non-burdensome call schedule.

3 clinic rooms and a procedure room

State-of-the-art ultrasound equipment on site

Staff includes 1-2 clinical staff members and a receptionist

1 Board Certified Gynecologist on staff in the building, 2 OB Nurse Practitioners, and 1 FP/OB Physician also on site.

12 Family Practice providers on site

Estimated Credentialing Timeframe: 45-90 days

Accepting recent/upcoming OB Fellowship graduates who are BE (must have plans to be BC within 1 year of employment) or BC in Family Medicine

Not accepting Locums candidates

Not accepting visa candidates

Cannot accept providers who have any malpractice cases that are in Probate or that require supervision. Clean malpractice is preferred.

All of the following are required at Name Clear unless noted otherwise:

Board Eligible (with plans to become BC within 1 year of employment) or Board Certified in Family Medicine - REQUIRED

IA license (or willing to obtain) - REQUIRED

OB Fellowship-trained (to be completed prior to start) - REQUIRED

Provider is able and interested in doing C-sections (state interest in "Additional Submission Details" field) - REQUIRED

Clean Malpractice - PREFERRED

Does the provider require visa support - J1 or H1B? (note in "Additional Submission Details" field) - REQUIRED

Certification Requirements:

State License Requirements:

• Iowa

Additional License Requirements:
