You will play a central role in supporting patients through withdrawal management and residential treatment, ensuring their medical needs are met with skill, compassion, and clinical excellence.Responsibilities include:Provide comprehensive primary care services, including medical histories, physical examinations, health assessments, diagnosis, treatment, and management of acute and chronic medical conditionsProvide initial and ongoing medical assessment and management of patients receiving treatment for substance use disorders, including withdrawal management and residential treatment servicesPrescribe medications and treatments within the APRN's scope of practice, applicable Connecticut law, organizational policies, and clinical protocolsMonitor treatment effectiveness, medication response, side effects, and changes in medical status; modify care plans as clinically indicatedPerform medication reconciliation and review patient medication regimens to promote safe, effective, and appropriate medication useIdentify medical conditions requiring specialty consultation, emergency evaluation, transfer, hospitalization, or a higher level of care and coordinate appropriate actionCoordinate referrals to primary care, specialty care, hospitals, community health centers, pharmacies, laboratories, and other community-based medical resourcesAssess medical stability and appropriateness for admission, continued stay, transfer, or discharge in collaboration with the medical and nursing teamCollaborate with physicians, nurses, counselors, recovery support staff, and administrative staff to coordinate patient careCommunicate effectively with outside healthcare providers, hospitals, pharmacies, and referral partners to support continuity of careParticipate in interdisciplinary team meetings, case conferences, care reviews, and discharge planning activities as appropriateMaintain accurate, timely, and complete clinical documentation in accordance with organizational, regulatory, accreditation, payer, and professional standardsWhat You'll Need:Master's degree or Doctor of Nursing Practice degree from an accredited nurse practitioner programGraduate training and current certification as a Family Nurse Practitioner or other appropriate primary care nurse practitioner certificationTwo or more years of experience in primary care, family medicine, internal medicine, urgent care, community health, correctional health, addiction medicine, withdrawal management, or residential treatment preferredExperience or demonstrated interest in addiction medicine, medications for substance use disorders, harm reduction, withdrawal management, and recovery-oriented medical care preferredCurrent unrestricted Connecticut Registered Nurse licenseCurrent unrestricted Connecticut Advanced Practice Registered Nurse licenseCurrent Basic Life Support certificationConnecticut Controlled Substance Registration and DEA registration required if prescribing controlled substances is part of the roleCommitment to maintaining all required continuing education, licensure, certification, and professional requirementsCommitment to SCADD's values of diversity, equity, inclusion, and trauma-informed practiceWhy SCADD?Since 1966, SCADD has remained at the forefront of substance use disorder treatment, continuously evolving to meet the needs of individuals, families, and communities across Southeastern Connecticut.
