Job Description

Addison Group is partnering with our client to hire a Palliative Care Support Coordinator . This role provides essential administrative, clinical, and operational support to a multidisciplinary palliative care team. The position is fully office‑based and interacts with patients and families exclusively by phone , helping coordinate care across home settings, senior living communities, skilled nursing facilities, and outpatient environments.

Key Responsibilities

  • Clinical Coordination — Assist with preparing patient charts, updating medical information, reviewing medication lists, and supporting care planning activities.
  • EMR Management — Enter patient data, demographics, and care plans into the EMR; ensure documentation is complete; generate reports and correspondence as needed.
  • Scheduling Support — Arrange evaluations, admissions, and follow‑up appointments; maintain accurate calendars for providers and admission nurses.
  • Administrative Operations — Handle intake calls, process referrals, manage records, assist with billing tasks, and complete general clerical duties.
  • Communication & Outreach — Provide updates to physicians and referral partners; respond to family inquiries; send educational materials; verify insurance benefits; and follow up on pending referrals.
  • Care Team Support — Assist with obtaining outside medical records, coordinating with pharmacies, and supporting nurses with patient intake and documentation needs.
  • Compliance & Professionalism — Serve as a compassionate, knowledgeable point of contact while maintaining strict confidentiality and adherence to HIPAA standards.
  • Special Projects — Contribute to department initiatives and additional tasks that enhance team efficiency and patient experience.

Qualifications

  • Certified Medical Assistant (CMA, RMA, NCMA, NRCMA, or equivalent)
  • Current AHA Basic Life Support certification
  • At least one year of medical assisting experience preferred
  • Strong multitasking ability and comfort managing high call and referral volume
  • Familiarity with medical terminology and EMR systems
  • Knowledge of Medicare/Medicaid benefits is a plus
  • Experience in palliative care, hospice, or home health is highly preferred

Additional Details

This role does not require travel and does not supervise staff. All patient and family interactions occur remotely via phone or electronic communication.
