PHYSICAL THERAPIST (PRN)

Physical TherapistThe Physical Therapist must demonstrate the knowledge and skills necessary to provide care appropriate to the age of the patients served on his/her assigned unit. The individual must demonstrate the principles of growth and development over the life span and possess the ability to assess data reflective of the patient’s status and interpret the appropriate information needed to identify each patient’s requirements relative to his/her age specific needs, and to provide the care needed as described in the department’s policies and procedures.The Physical Therapist must be able to:Complete initial patient evaluationsAssure presence of physician order in medical recordComplete evaluation per physician order, including recommended treatments, making changes with physician approvalObtain thorough patient history through subjective interview and review of available objective informationDesign a specific plan of care following determination of patient needs and revises as appropriate through ongoing assessment during treatment of patient or with reports of progress from physical therapy assistantFormulate short term/long term goals based on results of evaluation, complete with time framesCommunicate treatment plan clearly and effectively to physical therapy assistantTreatment procedures follow established departmental policies and procedures, physician orders, and established plan of carePertinent objective and subjective information regarding treatment is documentedResponsible for care of equipment; economic use of supplies, linens and utilities and cleanliness of department equipment used in the treatment of patientCharges are accurately prepared, based on treatment providedSupervision of support staff and physical therapy assistantNotify others of changes in patient’s schedule or conditionInstruct staff in delegated tasksProvide appropriate feedback to staff regarding performanceDelegate tasks to support staff and physical therapy assistant based on State Practice GuidelinesCommunicate verbally and written with other hospital employees, physicians, patients/families, and other agenciesPhysician progress notes completed accurately and in a timely mannerContacts appropriate staff based on change of patient conditionPatient and family education is thorough and timely, with a written program provided as necessaryParticipate in care conference and in-servicesAttend as assigned per DirectorPresent information in an organized and fluent mannerHas established goals for patient and/or presentation of informationEnhance job function through personal and professional developmentDemonstrate activity to impact change within department and within other systems involved in job functionSeek professional development to benefit department and personal/professional growthStay current on new trends and developments in physical therapy by survey of literaturePresent to others in the department, information learned during continuing education and self-studyFollow policies and procedures at Cody Regional HealthMaintain confidentiality, upholds patient rights and freedomsAttend and contributes as appropriate to mandatory education, departmental staff meetings, continuing education and other in-servicesKnow and follow policies relating to infection controlConform to all other policies and procedures of the facility
