Registered Nurse - Home Health

Brief Description We have a great opportunity for a Home Health RN! Home Health RN This is a full‑time position, Monday‑Friday 8:00am‑5:00pm. The service area includes Greensboro, High Point, Kernersville, Winston‑Salem, Lexington; Forsyth and Upper Davidson Counties. CPR certification from the American Heart Association or American Red Cross Basic Life Support (BLS) is required. We are seeking a dynamic, quality‑driven Registered Nurse (RN) Case Manager to pave the way for outstanding care delivery for our population of home health patients. The ideal candidate will have a passion for providing exceptional, compassionate care, ensuring high‑quality outcomes and improving the overall patient experience. As a Home Health RN, you will be responsible for delivering patient care, coordinating with interdisciplinary teams, and driving continuous improvements in care quality within our home health program. Education: Current, active Registered Nurse (RN) license in North Carolina (required). Bachelor’s degree in Nursing (BSN) preferred. Experience: Minimum of 2 years of experience in nursing, with at least 1 year in a home health or community health setting preferred. Experience in leading or coordinating patient care is strongly preferred. Skills and Abilities: Strong clinical assessment and critical thinking skills. Excellent leadership, communication, and interpersonal skills. Ability to work independently while managing a caseload of home health patients. Knowledge of home health care regulations, including Medicare and Medicaid guidelines. Proficiency with electronic health records (EHR) and other healthcare software. Strong organizational and time management skills, with the ability to prioritize patient needs. Other: Valid state‑issued driver's license required.
