Registered Nurse - Home Health

Job Description

Job Summary:
Provide comprehensive home health nursing care to patients requiring assistance in their residential environment. Coordinate and deliver personalized patient care in accordance with physician orders and agency protocols.

Responsibilities:

Administer direct patient care during home visits, including wound care, assessments, and patient education.
Coordinate with physicians and interdisciplinary teams to develop and implement care plans.
Supervise and evaluate Home Health Aides, Licensed Practical Nurses, and Licensed Vocational Nurses.
Document clinical findings and patient interactions accurately using HomeCare HomeBase (HCHB) and Oasis as required.
Maintain compliance with travel, documentation, and safety policies, including on-call responsibilities.
Manage travel between patient homes within a 75-mile radius, ensuring timely and quality care delivery.

Qualifications:

Hold a valid RN license in the state of practice with no pending restrictions.
Have a minimum of 1 year of recent adult home health or hospice experience.
Proficient in Oasis documentation and HomeCare HomeBase EMR within the last 6 months.
Possess current CPR and BLS certifications.
Have reliable transportation, a driver's license, and appropriate auto insurance.
Willing to travel up to 75 miles from the assigned clinic.
