You will provide comprehensive medical services, foster lasting relationships with your patients, and collaborate with a high-functioning care team to drive optimal health outcomes. Provide direct patient care through office visits, telehealth, and occasional same-day appointments Develop personalized treatment plans that may include lifestyle interventions, supplements, hormone therapies, and prescriptions Educate and empower patients on preventive health and functional medicine principles Collaborate with internal health coaches, nutritionists, mental health providers, and clinical support staff Document visits in the EHR in a timely and compliant manner Participate in team meetings, quality improvement initiatives, and training sessions Support clinic operations by contributing to a positive, team-oriented environment Active Oregon license as a Certified Physician Assistant ( or eligibility to obtain ) Board certification in Family Medicine or Internal Medicine Strong interest in preventive, functional, and lifestyle medicine Proficient in using electronic health records Medical, Dental, Vision, and Life Insurance 401(k) Retirement Plan with Company Match Paid Time Off and Paid Holidays
