Job Title: LPN Post Acute LTC
Client: Covenant Health Inc
Facility: Penacook Place
Location: 150 Water Street Haverhill, MA 01830
Duration: 13 weeks (Possibility of extension)
Payrate: $45/hr on w2
Available Shifts:
-Day Shift 7a-3p
-Evening Shift 3p-11p
General Summary
Provides nursing care services consistent with the responsibilities of an LPN and in accordance with established nursing practices, facility policies, and procedures.
Principal Duties and Responsibilities
- Observe and report symptoms and changes in residents' conditions.
- Administer medications as prescribed by physicians.
- Perform treatments and assessments as ordered or required.
- Count narcotics and maintain accurate records.
- Maintain documentation reflecting resident condition, medications, treatments, and progress.
- Transcribe written and telephone physician orders.
- Place medication/drug orders.
- Assist with completion of nursing histories, assessments, MDS, and care plans.
- Assist with the orientation and supervision of nursing staff.
- Provide direct resident care as necessary.
- Follow recognized nursing practices and facility policies and procedures.
- Participate in ongoing in-service education.
- Assume Charge Nurse responsibilities when necessary.
- Prepare and coordinate CNA assignments.
- Contact physicians/MDs as needed.
- Schedule tests, laboratory work, X-rays, and medical appointments.
- Assist physicians during rounds and prepare charts for physician signatures.
- Check meal trays for accuracy.
- Directly supervise CNAs.
- Perform all required treatments and nursing assessments.
- Complete resident admissions and discharges.
- Notify MD/NP of changes in resident status.
- Notify family members/HCP of changes in treatment status.
- Complete nursing notes and other required documentation.
- Update and maintain resident care plans.
- Report to and follow direction from Nurse Managers.
Physical Requirements
- Ability to lift up to 50 pounds .
- Ability to push/pull up to 40 pounds .
- Ability to stand for up to 4 hours per day .
- Ability to bend, squat, and stoop to floor level.
- Ability to kneel and walk as required.
- Ability to reach heights of up to 4.5 feet .
- Ability to see and hear with or without prostheses.
- Ability to move intermittently throughout the workday.
- Ability to cope with physical and mental job-related stress.
Required Knowledge, Skills & Abilities
- Ability to recognize changes in residents' conditions and take appropriate action.
- Ability to read, write, and communicate effectively in English in a legible and understandable manner.
- Current CPR/AED certification .
- Strong clinical, communication, documentation, and supervisory skills.
Education & Licensure
- Graduate of an accredited school of nursing .
- Current nursing license in the State of Massachusetts as either:
- Registered Nurse (RN) , or
- Licensed Practical Nurse (LPN) .
