Wellness Nurse

Job Title

Clinical

• Performing nursing procedures within the scope of practice as allowed by state law and in compliance with Villas of Holly Brook.
• Ensuring sound infection control practices are in place that promotes a healthy environment.
• Contributing to resident assessments and customizing service plan development.
• Leading the medication administration process by correctly transcribing physician orders, correctly ordering medication and completing error free medication reminders.
• Providing first aid and emergency assistance while on duty within the community.
• Participating in resident and family care conferences.
• Maintaining all privacy rules and regulations pertaining to the Health Insurance Portability and Accountability Act (HIPPA)
• Ensuring that changes in resident condition are evaluated, monitored, documented and that effective interventions are implemented.
• Reviewing resident records on a regular basis and ensuring that documentation accurately addresses the healthcare issues, including identified concerns, interventions and outcomes, of all residents.
• Ensuring that resident care is provided in a safe and effective manner, while consistently maintaining resident dignity, choice, privacy, and respect.
• Maintaining a current knowledge of OSHA and state regulations and routinely monitoring community compliance with regulations-related to resident care and nursing practices.
• Any other duties assigned by Wellness Director.

Supervisory Functions

• Assisting with supervising Wellness Department employees in providing day-to-day care.
• Providing orientation and ongoing training as requested.
• Communicating and enforcing policies and procedures including Villas of Holly Brook policies and procedures dealing with rules of employee conduct, the employer's safety and sanitation policies and other policies.

Clinical Nurse Functions

• Scheduling podiatry visits, assist podiatry during the visit, if necessary, complete documentation in PCC
• Incident reports: Complete following an incident, notify PCP, provide documentation and timeline to Wellness Director following company policy for incident follow-up
• Scheduling
• Medications: check in medications, add or remove any medications based on changes, fax new orders to pharmacy, deliver medications as needed to resident apartments and place in locked cabinet, pre-fill insulin syringes, remove medications after resident moves out, weekly narcotics count, administer medications if resident is unable to do it themselves
• TB testing: administer 2-step TB for any employee or resident who needs it
• Obtain physician certifications
• Other duties as assigned by Wellness Director

Customer Service

• Performing job duties for residents and with team members in a courteous and professional manner.
• Taking initiative to ensure resident safety and satisfaction is a priority.
• Meeting and greeting visitors in a friendly, helpful manner (Visitors include anyone who visits the community; specifically, current residents' families and friends, prospective residents and their families referral sources, vendors and regulators).
• Answering phones appropriately, according to company and community standards.
• Communicating professionally and cooperating with Executive Director and all community personnel.

Residents' Rights

• Ensuring compliance and understanding of all regulations regarding residents' rights.

Training

• Attending all required in-service and training

Other

• Following & communicating Villas of Holly Brook policies and procedures.
• The job description provides a framework for the job; other duties may be assigned as necessary.

Position Requirement

• Licensed RN/LPN in good standing with State Licensing Authority.
• Experience of one year or more in the care of senior adults and adults with cognitive decline is preferred.
• Ability to communicate effectively with residents, families, staff, community and State officials and general public.
• Must have compassion for and desire to work with the elderly.
• Must demonstrate the ability to work responsibly as a team member as well as an individual.
• Must be honest, fair, dependable, respect confidentiality and the rights and privacy of others.
• Must practice and promote Villas of Holly Brook Policy and Procedures and Mission Statement.
• Must have exceptional organizational skills and ability to re-prioritize daily tasks in order to accommodate fluctuating needs of the resident and community.
• Must have strong documentation skills.
• Must pass criminal background check.
• Must meet all health requirements.
• Must be able to perform duties and responsibilities (essential job functions), with or without reasonable accommodations.

Physical and Mental Requirements

• Medium work; ability to exe1i 20-50 pounds of force occasionally, and/or 10-25 pounds of force frequently, and/or greater than negligible up to 10 pounds of force constantly to lift, carry, push, pull or otherwise move objects.
• Must be able to relate to and work with the ill, disabled, elderly, emotionally upset and at times hostile people within the community.
