Mental Health Counselor - Geriatric PACE Program

Element Care seeks a Psychotherapist or Licensed Mental Health Counselor to provide counseling and behavioral health support to frail seniors enrolled in our Massachusetts PACE program. In this role, you’ll conduct assessments, develop person-centered treatment plans, and deliver individual and group therapy to help participants manage depression, anxiety, grief, and adjustment to illness. You’ll collaborate closely with an interdisciplinary team—including medical, social work, and rehabilitation staff—to integrate mental health into comprehensive care plans.
