Anova Care is actively seeking a compassionate and motivated Physical Therapist (PT) to join our expanding Home Health team. This is a great opportunity for clinicians who are passionate about providing personalized one-on-one patient care while enjoying flexibility and autonomy in their schedule.

As a Home Health PT, you will help patients improve mobility, regain independence, and enhance their quality of life in the comfort of their own homes.

Responsibilities:

• Evaluate and assess patients physical therapy needs.
• Develop individualized treatment plans.
• Provide therapeutic exercises and rehabilitation services.
• Educate patients and families on treatment plans and mobility improvement.
• Coordinate care with physicians and interdisciplinary team members.
• Complete timely and accurate documentation.

Qualifications:

• Active Colorado Physical Therapist (PT) license
• Home Health experience preferred, but not required
• Strong communication and patient care skills
• Reliable transportation preferred
