Schedule: 1:1 Patient Ratio | (8:00 PM 8:00 AM), Must include 2 weekend nights per month

Coverage Area: Manatee, Charlotte, DeSoto, and Sarasota counties. Mileage Reimbursement

What You'll Do

Under the direction of the RN case manager, provides continuous bed-side clinical observations, prescribed care, physical, and emotional support to patients as directed in plan of care, reporting significant changes in patient condition and family needs as needed.

In collaboration with the RN, accepts and processes physician orders to maximize patient comfort.

Continuous Care LPNs also assist Triage and Care Centers overnight when Continuous Care cases are pending.
