Clinical pharmacists are responsible for improving the patient’s experience, safety, and achieving positive clinical and financial outcomes. This position is responsible to an immediate supervisor/manager.I. Major Responsibilities: The following key performance areas (Clinical, Operations/Distribution, Financial, Safety and Regulatory, Research and Education) are performed by the clinical pharmacist:Clinical Performance 1. Provides medication management services in a collaborative manner that consist of the following: a. Review patient medication history, educate patients on drug therapy, establish desired outcomes, develop/modify therapeutic plans, determine alternatives for achieving outcomes, develop drug monitoring plans, monitor patient outcomes, document medication related issues in electronic health record, interpret laboratory data, review patient medication profiles, monitor pharmacokinetics of drug therapy, monitoring for potential adverse drug – drug, drug-lab, drug-disease state, and drug - food interactions.2. Maintains and updates a patient profile with demographics (e.g. height and weight), diagnosis, allergies, and current medications. 3. Monitors drug therapy to evaluate appropriateness of use, dose, dosage form, regimen, route, therapeutic duplication, and drug interaction. 4. Provides drug information to health care professionals and to patients. 5. Documents clinical interventions. 6. Counsels patients on discharge medications as appropriate. 7. Performs clinical review and verification of medication orders. 8. Performs medication order entry when needed. 9. Discusses medication order clarifications with the prescriber, documents interventions, and communicates medication order changes as needed. 10. Facilitates medication discharge planning and smooth transition to alternate sites of care and access to an ambulatory pharmacy.Operations/Distribution Performance 1. Ensures the timely and accurate dispensing of medications from the pharmacy area. 2. Ensures proper technique and accurate preparation of all pharmaceutical products, including oral, non-sterile and sterile compounding, chemotherapeutic, hazardous, and investigational agents. 3. Responsible for the accurate filling and dispensing of medications from the pharmacy area. 4. Cross-covers in a variety of practice areas within the pharmacy as requested by supervisor. 5. Ensures regulatory compliance of controlled substance distribution and monitoring. 6. Supervise work of pharmacy technicians. 7. Provide feedback to maintain and optimize pharmacy technology.Financial Performance 1. Promotes the use of formulary drugs by converting non-formulary orders to formulary drugs when possible according to P&T protocol. Coordinates procurement of non-formulary drugs when necessary. 2. Conducts targeted drug program and drug usage evaluations as needed and reports results to Department Managers/ Pharmacy and Therapeutics Committee. 3. Identifies and acts upon cost avoidance in drug therapy by promoting cost effective regimens, drugs, and by reducing waste. 4. Facilitates cost saving programs such as IV to PO and therapeutic substitution. 5. Manages medication inventory, expiring products, and recalls.Safety and Regulatory Performance 1. Detects, monitors, documents, and reports adverse drug reactions and medication errors. 2. Utilizes drug use reports to identify adverse drug events. 3. Participates in Joint Commission and other accreditation and regulatory activities such as National Patient Safety Goals and Department of Pharmacy Quality Improvement Plan.Research and Education Performance 1. Precepts Intermediate Pharmacy Practice Experience (IPPE) rotation students, and Advanced Pharmacy Practice Experience (APPE) rotation students, and pharmacy residents consistent with academic credentials and clinical experience. 2. Participates in continuing education programs, meetings, training programs and related activities. 3. Prepares educational materials and participates in information sharing activities for the health care community and general public.II. Position Qualifications:License/Certification/Education: Required: 1. Doctor of Pharmacy (Pharm.D) degree; OR Bachelor of Science in Pharmacy (BSPharm) degree with 3 years hospital experience. 2. Licensure with the Massachusetts Board of Registration in Pharmacy.Preferred: 1. ASHP Accredited Residency in Pharmacy Practice (PGY1) is recommended. 2. Pharm.D. degree with minimum of 1 year of hospital experienceUnless certification, licensure or registration is required, an equivalent combination of education and experience which provides proficiency in the areas of responsibility listed in this description may be substituted for the above requirements.ADDENDUM FOR CLINICAL PHARMACIST, TRANSITION OF CARE ROLEPosition Summary: The Transition of Care (TOC) clinical pharmacist supports the medication reconciliation process and provides medication management services that integrate clinical services, order management, patient/staff education, and research. The TOC pharmacist is responsible for improving the patient’s experience, safety, and achieving positive clinical and financial outcomes upon hospital admission. Participates in and contributes to educational programs, committees, and clinical research.Major Responsibilities: 1. Reviews completeness, accuracy, and clinical appropriateness of home medication lists collected for patients being admitted to the hospital. Supports the medication reconciliation process, which includes identifying and reviewing electronic and paper health records and medication histories, interviewing patients on home drug therapy, and contacting facilities for medication information (i.e. pharmacies, hospitals, long-term care facilities, physician offices, etc. All other responsibilities to be included from Clinical Pharmacist Job DescriptionDifference in Qualifications: 1. Responsible for improving the patient’s experience, safety, and achieving positive clinical and financial outcomes. Responsible for all aspects of medication safety at both inpatient and outpatient settings which includes monitoring current practice, identifying opportunities for improvement, facilitation of error prevention strategies, monitoring impact of improvement strategy and developing/revising policies and procedures. Focuses on best practices for the patient and staff, as well as the education/training of all applicable staff.Major Responsibilities: 1. Works with Pharmacy, Risk Management, Patient Safety, Medical and Nursing staff and other Healthcare providers within UMass Memorial Medical Center to research systemic organizational vulnerabilities and analyze the human, organizational, and systemic factors which contribute to adverse medication occurrences. 2. Collects, reviews, and investigates potential and actual adverse medication events including medication occurrences, adverse drug reactions, and quality improvement data (e.g., smart pump event data, triggers and surveillance information, automated dispensing system, bar code scanning reports), and recommends actions steps to prevent future events. 3. Shares information about actual or potential medication errors or harm with safety organizations such as the Institute for Safe Medication Practices (ISMP), the FDA, drug or product manufacturers, and state error reporting programs.
