What You'll Do as the Housing Specialist:
This is a part-time position. Day shift.


Primary duties:

• Help clients gain access to needed medical, social educational and other services.
• Provide direct assistance in gaining access to services, coordination of care, oversight of the entire case and linkage to appropriate services.

Areas of service:

• Assessment of the eligible individual to determine housing needs
• Responsible for accurate and timely documentation related to grant deliverables
• Referral and related activities to advocate and assist the individual obtain needed services
• Monitoring and follow up of the housing plan
• Linkage and networking with court, health care providers, employers and other community agencies
• Provide integration and assistance to clients and families to help sustain within the community/least restrictive environment
• Accountable for the flow of information within a multidisciplinary team setting
• Special Populations/Skills/Employment duties, as assigned.

Requirements:

• Associate degree required with two years of related and relevant experience required;
• BA/BS preferred.
• Experience with targeted population highly preferred.
• Valid driver's license
• Proof of insurance
