Join our dynamic healthcare team as an In-Home Physical Therapist on a per diem basis, where you will play a vital role in delivering personalized, high-quality physical therapy services directly to patients in their homes and community settings. This flexible position offers the opportunity to make a meaningful difference in patients’ lives by providing expert care tailored to their unique needs. You will assess, plan, and implement effective treatment strategies across diverse populations, including geriatrics, orthopedics, and neurological ensuring optimal recovery and improved quality of life. Opportunity for growth into part-time/full-time roles. Responsibilities Conduct comprehensive patient assessments to develop individualized care plans that address specific mobility and functional needs. Deliver expert physical therapy interventions such as balance/gait training, reducing falls, orthopedics treatment, and rehabilitation exercises in home and community environments. Manage discharge planning processes to ensure seamless transitions from hospital or inpatient settings to home care. Collaborate with multidisciplinary teams to coordinate care plans that incorporate patient goals and medical requirements. Educate patients and caregivers on physical therapy techniques, injury prevention, and health maintenance strategies. Utilize electronic medical records (EMR) systems accurately to document patient progress, treatment plans, and clinical notes. Monitor patient progress regularly, adjusting care plans as necessary to optimize outcomes across outpatient clinics, hospital stays, or home health visits. Qualifications Valid license as a licensed Physical Therapist with current registration in the applicable jurisdiction. Demonstrated experience in inpatient hospital settings, outpatient clinics, or home health environments. Proven ability to deliver effective patient care across diverse populations including orthopedics, neurological, and geriatrics. Familiarity with EMR systems for documentation and care coordination. Excellent communication skills to educate patients and collaborate with healthcare teams effectively. Interest in geriatrics, Parkinson's, and neurologcal patients. #J-18808-Ljbffr
