How you’ll make an impact in this role

• Lead the coordination of multidisciplinary teams to ensure every patient receives a seamless, well-integrated rehabilitation experience .

• Create and adapt personalized patient care plans that directly drive recovery goals and improve long-term health outcomes .

• Support patient safety and clinical excellence by monitoring conditions and communicating vital updates to the care team in real-time .

• Guide patients and their families through the discharge planning process to ensure a confident and safe transition back to their community .

What minimum requirements you’ll need

Licensure / Certification / Registration:

• BLS Provider obtained within 1 Month (30 days) of hire date or job transfer date required. American Heart Association or American Red Cross accepted.
• Licensed Registered Nurse credentialed from the Kansas Board of Nursing required.

Education:

• Diploma from an accredited school/college of nursing OR professional licensure at time of hire required.
