Care Advocate For Older And/Or Disabled AdultsAdvocate for older and/or disabled adults and their loved ones to help make decisions related to healthcare, aging and long-term care planning which will:Reduce hospitalizationsImprove quality of lifeDecrease the burden on the family or responsible partyRecognize and advocate for the client's values and wishes, rather than your ownConduct, develop, monitor, and manage care plan and assessments to identify:Issues and problemsEligibility for assistanceNeed for servicesOnboard and sign up new clients, educating them on how our Care Managed Home Care (CMHC) services will help them age in placeReview financial, legal and/or medical issues and offer referrals and recommendations to specialists and other professionals focusing on avoiding future problems and conserving assetsAssist with determining the level of care needed and moving a client to a more appropriate facility or the arrangement of in-home careAct as liaison to responsible parties and professionalsFacilitate our Care Managed Home Care model by:Manage intakes, complete on-site safety assessments and complete care plans for new clientsProvide coaching for caregivers with new clients to ensure successOverseeing the orientation and training to new and existing caregiversComplete documentation requirements accurately and timelyParticipate in marketing efforts with the goal of educating the public, cultivating clients, and securing new clientsCompensation: $30.00 - $35.00 per hourAging Life Care Professionals® offer a holistic, client-centered approach to caring for older adults or others facing ongoing health challenges.
