Sapphire Health Services is currently seeking a dedicated individual to join our team and uphold our commitment to excellence.We are seeking a compassionate and dependable Caregiver to join our Assisted Living and Memory Care community. In this role, you will provide hands-on assistance with daily living activities, ensure resident safety, and foster a positive, engaging atmospheres for all residents, including those receiving specialized support in our Memory Care community.Responsibilities include:Providing assistance with activities of daily living (ADLs).Providing companionship, emotional support, and meaningful engagement to residents.Assisting in maintaining accurate records of care provided, including daily logs, progress notes, and incident reports.Maintaining resident confidentiality.Encouraging clients to participate in activities that promote physical, mental, and emotional well-being.Supporting clients in maintaining independence in their daily routines and decision-making.Ensuring a safe, clean environment by identifying and addressing potential hazards in community settings.Responding promptly and appropriately to emergencies, such as falls, injuries, or sudden health changes, following established protocols.Collaborating with the care team, including healthcare professionals, social workers, and family members, to provide coordinated care.Communicating effectively with residents, families, and team members to ensure clarity and understanding of care plans and any changes.Presenting for staff meetings and training.Monitoring and documenting changes in residents' physical, emotional, or cognitive conditions and promptly reporting concerns to nursing staff or supervisors.Assisting residents during mealtimes, including serving meals, encouraging proper nutrition, and providing feeding assistance when needed.Following individualized care plans and complying with state regulations, company policies, and infection control protocols.Other duties as assigned.Requirements include:Must be 18 years of age NAC/CNA or NAR or HCA acquired within 120 days of hire.High school diploma or equivalent required.Previous experience in caregiving, home care, or community-based care is preferred.Ability to work independently and as part of a team.Patience, empathy, and a genuine desire to help others.Good organizational and time management skills.Ability to perform physical tasks such as lifting, bending, and assisting residents with mobility.Must be able to stand, walk, and reach for extended periods.Ability to pass a background check.
