What You'll Do

• Provide skilled physical therapy services in patients' homes
  
• Complete evaluations and develop individualized plans of care
  
• Help patients improve mobility, strength, safety, and independence
  
• Educate patients and families on therapy goals, equipment use, and injury/disease management
  
• Monitor patient progress and adjust treatment plans as needed
  
• Collaborate with physicians, PTAs, nurses, and the interdisciplinary care team
  
• Complete documentation in accordance with agency and Medicare guidelines
  
• Support continuity of care through admissions, transfers, and discharges
  
  
  
  

Qualifications

• Current Physical Therapist license in the state of employment
  
• Minimum of 1 year of PT experience in an acute care setting required
  
• Home health experience preferred
  
• Valid driver's license and auto insurance
  
• Strong communication, time management, and clinical judgment skills
  
• Comfortable working independently while staying connected to a supportive team
